چکیده

my viab Several case studies of acute transient left ventricr apical ballooning without significant coronary ry disease (takotsubo cardiomyopathy) have been orted. The pathophysiologic mechanism of this drome is believed to be related to catecholamineuced stunning of the apical myocardium. Definitive of, however, is lacking. Here we report a case of transient apical ballooning which metabolic imaging with positron emission ography (PET) revealed profoundly reduced fluo-18 fluorodeoxyglucose (FDG) uptake in the balned apical wall with relatively normal perfusion, a ern consistent with myocardial stunning. This perfu-metabolism pattern normalized, as did contractility. Case report. A 52-year-old white woman preted to the emergency department 8 hours after the te onset of chest pain and shortness of breath. ptoms started and then waxed and waned after an tional argument with her husband. She denied toco, alcohol, or drug use. Her past medical history was y significant for hyperlipidemia, for which she had n taking atorvastatin. On arrival, her blood pressure 77/50 mm Hg and her heart rate was 88 beats/min. ular venous distension was appreciated in the sitting ition, and respiratory crackles were heard bilaterally. murmurs, rubs, or gallops were appreciated. The trocardiogram revealed mild ST elevation in leads II, and aVF with diffuse T-wave inversion and QT longation (Figure 1). A chest radiograph was consiswith bilateral pulmonary parenchymal congestion. ergent coronary angiography revealed normal coroy arteries. Left ventriculography showed distal ante, apical, and distal inferior akinesis with hyperkinesis the remaining walls (Figure 2). The level of serum

Diagnostic utility of 123I-BMIPP imaging in patients with Takotsubo cardiomyopathy.

OBJECTIVE Takotsubo cardiomyopathy is a cardiac syndrome with an acute onset defined by chest symptoms and ST segment elevation on electrocardiograms. Takotsubo cardiomyopathy is sometimes misdiagnosed as acute myocardial infarction (AMI). Therefore a non-invasive diagnostic method is needed to be established for setting up appropriate strategies. The purpose of this study was to detect myocard...

Impairment of cardiac metabolism and sympathetic innervation after aneurysmal subarachnoid hemorrhage: a nuclear medicine imaging study

INTRODUCTION Although aneurysmal subarachnoid hemorrhage (SAH) is often complicated by myocardial injury, whether this neurogenic cardiomyopathy is associated with the modification of cardiac metabolism is unknown. This study sought to explore, by positron emission tomography/computed tomography, the presence of altered cardiac glucose metabolism after SAH. METHODS During a 16-month period, 3...

Insights into coronary artery disease gained from metabolic imaging.

Positron emission tomography offers the possibility of evaluating and quantifying regional myocardial blood flow and metabolism. Used in patients with coronary artery disease, positron emission tomography has demonstrated sustained metabolic activity in regions with reduced blood flow and impaired contractile function, and it thereby enables differentiation between viable myocardium and myocard...

Takotsubo Cardiomyopathy and Coronary Artery Disease: A Meaningful Coincidence?

S ince its initial description , Takotsubo cardiomyopathy has generated much debate and interest. Over the past 25 years, much progress has been made in understanding this syndrome. While the clinical presentation and characteristics have been well described and agreed upon, the etiology is a bit more of a debate. At present, the most commonly accepted theory on the pathophysiology of Takotsubo...
